About The Position

The Healthcare Manager at WellMed, part of the Optum family, is responsible for supporting patients with high-risk health conditions in navigating the healthcare system. This role involves patient education, resource advocacy, and collaboration with multidisciplinary care teams to enhance patient empowerment and ensure a high quality of life. The Healthcare Manager operates in a self-directed environment, performing nursing duties within the scope of an RN license and contributing to the overall improvement of healthcare delivery.

Requirements

  • Bachelor's degree in Nursing or Associate's degree in Nursing with 4+ years of experience as an RN.
  • Registered Nurse with an active and unrestricted license to practice in the state of employment.
  • Current BLS certification.
  • 2+ years of experience in a physician's office, clinical, or hospital setting.
  • Knowledge of chronic diseases, especially COPD/asthma, diabetes, CHF, and IHD.
  • Proficient computer skills for electronic medical records.
  • Proven ability to interact productively with individuals and multidisciplinary teams.
  • Excellent verbal and written communication skills.
  • Solid interpersonal skills and excellent organizational and prioritization skills.

Nice To Haves

  • Experience in patient education and motivational interviewing skills.
  • Fluent written and verbal skills in English and Spanish.

Responsibilities

  • Identify patients at high risk in collaboration with providers and clinic staff.
  • Support longitudinal care of patients with chronic conditions through assessments, medication reconciliation, and patient education.
  • Assist in the transition of patients from inpatient to outpatient settings by assessing transitional needs and providing education.
  • Coordinate with providers to establish or update patient care plans.
  • Document accurately and timely in the electronic medical record.
  • Participate in daily huddles and Patient Care Coordination meetings.
  • Prepare reports for weekly meetings.
  • Maintain continued competence in nursing practice and knowledge of current evidence-based practices.
