About The Position
As an RN Health Coach, you will be working one on one with SoonerCare members to provide education, support, and self-management tools aimed at improving the member’s health. You will assist primary care practices to build sustainable, quality-driven care delivery processes through the evaluation and coaching of patients with chronic medical conditions. You will oversee data capture and analysis to identify gaps in care delivery, implementing and teaching quality improvement (QI) methods to close gaps and improve the practice’s ability to effectively manage its patient population through data analysis and process redesign. May provide psychosocial assessments, crisis intervention, and counseling to members using brief solution-focused therapy to guide them to overall wellness. Must reside in Oklahoma.
