About The Position

The RN Field Coordinator (RN FC) plays a crucial role in ensuring continuity of care for nursing home residents by facilitating effective onboarding of new health plan members and collaborating with local clinical staff. This position involves data gathering, training nursing facilities on clinical models, and supporting the National Care Coordination Team in addressing member needs. The RN FC will also assist in telehealth services and conduct orientation sessions for nursing homes, ensuring a smooth transition for new members.

Requirements

• Licensed registered professional nurse in good standing.
• Valid state driver's license with a good driving record and proof of automobile insurance.
• Associate degree in nursing required; bachelor's degree preferred.
• Minimum 2 years of experience as a Registered Nurse.
• Excellent verbal and written communication skills.
• Proficient in EMR/clinical platforms and MS Office products.
• Knowledge of challenges in serving the long-term nursing home population.
• Managed care coordination background/experience.

Nice To Haves

• Nursing home background or health plan case management experience preferred.

Responsibilities

• Support continuity of care data gathering and facilitate onboarding of new health plan members.
• Work with clinical staff to educate and train nursing facilities on the clinical model.
• Access facility and LH EMR data to understand member transition needs.
• Communicate member needs to the National Care Coordination Team.
• Facilitate access to external appointments for members and communicate back to the member's NP and NH contact.
• Perform care coordination activities, including member transitions of care communication and assisting with prior authorization requests.
• Participate in Longevity Health NC standing clinical meetings.
• Assist with data gathering for hospitalization review sessions.
• Conduct nursing home orientation sessions for all shifts on LH MOC and introduction to the model.
• Assist with LH EMR data input as needed.
