As a RN Field Clinical Care Coordinator with UnitedHealth Group, you’ll wear many hats and work in a variety of environments. This is a Field Based position with a Home-Based office. If you are located in or within commutable driving distance to the Southeast Houston, TX area, you will have the flexibility to work a Hybrid/Field schedule as you take on some tough challenges. Primary Responsibilities: Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction Support members with condition education, medication reviews, and connections to resources such as Provider Services, Private Duty Nursing, DME, and supplies Assist members with the transition from a care facility back to their home This role involves approximately 75%25 travel, and you'll need to be comfortable dealing with a full range of in-home and facility-based environments Most of the time you'll work autonomously, so the ability to stay focused and motivated is essential This function is for UHC C&S Only (with the exception of the Community Health Worker role) Positions in this function address efforts focused on medical, behavioral, and social determinants of health factors that improve the well-being of individuals, families, and communities Generally, work is self-directed and not prescribed Works with less structured, more complex issues Serves as a resource to others What are the reasons to consider working for UnitedHealth Group? Requirements

  • Associate's degree (or higher)
  • Current unrestricted RN license
  • 3+ years of clinical experience in a hospital, acute care, home health/hospice, direct care, or case management
  • Intermediate level of computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
  • Valid driver's license, access to reliable transportation and the ability to travel up to 75%25 in an assigned region to visit Medicaid members in their homes and/or other settings
  • Reside in Houston, TX area

Nice To Haves

  • Case management experience including Certification in Case Management
  • Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
  • Experience in Home & Community based on Long-Term Care services delivery
  • Home care/field-based case management
  • Bilingual skills
  • Reside in the SE Houston, TX area
