As a Field-Based RN Case Manager with UnitedHealth Group, you'll wear many hats and work in a variety of environments. This is a Field Based position with a Home-Based office. If you are located in Southeast Houston, TX market you will have the flexibility to work a Hybrid/Field schedule as you take on some tough challenges.

Requirements

  • Associate's Degree (or higher)
  • Current, unrestricted RN TX license
  • 3+ years of clinical experience in a hospital, acute care, home health/hospice, direct care, or case management
  • Intermediate level of computer/typing proficiency to enter/retrieve data in electronic clinical records; experience with email, internet research, use of online calendars and other software applications
  • Access to reliable transportation & a valid US driver's license

Nice To Haves

  • Case management experience including Certification in Case Management
  • Experience working with the needs of vulnerable populations who have chronic or complex bio-psychosocial needs
  • Experience in Home & Community based or Long-Term Care services delivery
  • Home care/field-based case management
  • Bilingual skills
  • Reside in or within commutable driving distance to Belton, Harker Heights, Killeen, Westcliff or Woodland, TX

Responsibilities

  • Provide a complete continuum of quality care through close communication with members via in-person or on-phone interaction
  • Support members with condition education, medication reviews, and connections to resources such as Provider Services, Private Duty Nursing, DME, and supplies
  • Assist members with the transition from a care facility back to their home
  • Work autonomously and stay focused and motivated
  • Address efforts focused on medical, behavioral, and social determinants of health factors that improve the well-being of individuals, families, and communities
  • Serve as a resource to others
