About The Position

It is our mission to provide uncompromising, compassionate, end-of-life care to patients of all ages and their families. Serving 13 counties that surround the Houston area, we are committed to being the highest quality hospice provider wherever we serve. At Houston Hospice, we focus on caring, not curing. We take a team approach to care that includes physicians, nurses, social workers, chaplains, hospice aides, volunteers, and bereavement counselors. We provide hospice service wherever patients and families need us: in their home, in a long-term care facility, or in our inpatient care unit that is based in the Texas Medical Center. Our team of doctors and nurses specialize in managing pain and symptoms that are unique to life-limiting illnesses. Summary: We are seeking a candidate who would be responsible for the overall management of a patient's case. Responsibilities include coordination of care with the client, family, and other providers.

Requirements

  • Graduate of an accredited school of nursing with current Texas license.
  • 1+ years of hospice experience or experience in a hospital setting is required.
  • Current certification in basic life support.
  • Ability to function as part of interdisciplinary team.
  • Strong interpersonal skills – able to communicate with team members and patients from a diverse community.
  • Reliable transportation and/or reliable insured vehicle and valid Texas Driver’s License.
  • Ability to travel to multiple work locations throughout the day.

Nice To Haves

  • Bachelor’s degree in nursing preferred.
  • Knowledge about hospice industry and experience in a hospice or hospital setting is strongly preferred.
  • Experience using a clinical information system is preferred.

Responsibilities

  • Provides direct nursing care case management work appropriate for a licensed RN.
  • Participates in the development of the patient’s plan of care and coordinates the services and activities of the multidisciplinary team in respect to the plan of care
  • Documents activities and assessments in the clinical information system.
  • Provides support to patient’s family/friends including education, expectations, and reporting changes in condition
  • Understands and is compliant with organizational policies and processes relating to regulatory, accreditation and privacy standards
  • Performs other duties as required and/or requested by management
