Family Medicine at Richmond Clinic is a primary care Family Medicine clinic and Federally Qualified Health Center (FQHC). The patient population includes pregnant people, babies, children, adolescents, adults, and geriatric patients. The patients are generally moderately stable to moderately unstable, with complex medical conditions and SDOH barriers that require medical and nursing interventions. These interventions include oral and injectable medications, foot care, telephone and in-clinic triage, care coordination, health education, complex wound care, pain management, and behavioral and/or crisis management. Nurses perform patient visits, telephone triage and advice, and manage an in-basket workload in EPIC where patients are reviewed for Care Management, Care Coordination, follow-up for discharge planning, and triage/advise for medical conditions. Care Coordination and Disease Management are a high priority aspect of the job. Nurses work in pods within a care team of Providers, Pharmacists, and Behavioral Health clinicians.
