About The Position

The Clinical Nurse (CN) Facility Resource is responsible for providing a high level of clinical support to the acute care areas and numerous procedural areas within Community Health System (CHS). This position delivers patient care through assessment, planning, implementation and evaluation of patient needs. Additionally, will collaborate with interdisciplinary teams, mentor / coach staff members and provide unit based education and support on processes.

Requirements

  • High School Diploma, High School Equivalency (HSE) or Completion of a CHS Approved Individualized Education Plan (IEP) Certificate required
  • 2 years of nursing experience of which at least 1 year in an Intensive Care Unit (ICU) or Emergency Department required
  • RN - Current and valid Registered Nurse license to work within the state of California required
  • BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required
  • ACLS - Advanced Cardiac Life Support Certification required
  • PALS - Pediatric Advance Life Support Certification (CCMC and CRMC only) required within 180 days of hire or transfer into position
  • One of the following is required within 180 days of hire or transfer into position: o CCRN - Certified Critical Care Registered Nurse o CEN - Certified Emergency Nurse o NATIONAL - National Certification related to specialty and applicable to role

Responsibilities

  • Rounds on high risk patients that are identified through rounding and early warning scores (including Sepsis Surveillance) within EPIC for all inpatients with a primary focus on patients outside the Emergency Department (ED) and ICUs.
  • Assists with various procedures including but not limited to CVAD access and removals (ports), central line and dialysis line insertions and removals, I/O insertions and removals, ultrasound IV starts with CN teaching, level one infusions and procedural sedation.
  • Responds to all Code Blues, Code Whites, OB Heme alerts, Code Strokes and Rapid Responses. Validates patient decompensation prior to calling rapid responses.
  • Provides nursing care, including medication administration, to patients who are pending transfer to higher level of care.
  • Supports emergent/urgent monitored transports to ICUs, SDU, CPCU, MRI, Ultrasound, IR and CT.
  • Provides mentorship and coaching to clinical staff.
  • Consults with department leadership and team members to support and educate on new or updated processes.
  • Performs miscellaneous job related duties as assigned.
