Responsibilities

  • Perform social work assessments and interventions for inpatients, outpatients, and emergency department patients.
  • Engage in patient and family-centered care that values personal self-determination.
  • Manage multiple priorities and engage in creative, compassionate, and ethical problem-solving.
  • Perform comprehensive assessments and develop treatment plans integrating medical, social, and resource issues.
  • Work collaboratively with healthcare team members to develop and implement treatment plans.
  • Conduct crisis intervention, patient/family intervention, high-risk screening, and brief counseling.
  • Refer patients for financial or other identified resource needs.
  • Arrange and facilitate family/patient representative meetings with the healthcare team.
  • Assist in post-acute placement on complex discharges and engage appropriate agencies or community resources.
  • Establish and maintain professional relationships with physicians, case managers, nursing staff, county agencies, community resources, patients, and families.
  • Serve on hospital committees and participate in developing and presenting in-services to hospital staff.
  • Abide by all Joint Commission Requirements including cultural diversity, patient care, patients' rights, and ethical treatment.

Requirements

  • At least one (1) year of clinical and/or medical social work experience required.
  • Knowledge of contemporary behavioral and systems theories relevant to health care.
  • Understanding of end-of-life dynamics and interventions, grief and bereavement counseling.
  • Ability to identify substance abuse and provide interventions.
  • Support for victims of abuse, neglect, or violence.
  • Knowledge of community or financial resources for underserved or vulnerable populations.
  • Thorough comprehension of OBRA and Title 22.
  • Excellent customer service and presentation skills.
  • Strong interpersonal and written communication skills.
  • Demonstrated ability to apply analytical and problem-solving skills.
  • Ability to manage multiple tasks or projects effectively.
  • Ability to work independently with a high degree of detail orientation.
  • Ability to work efficiently in a fast-paced environment with changing priorities.
  • LIC LSW = Licensed Social Worker-State.
