About The Position

Position Overview: The RN Emergency Department (ED) Case Manager works with patients and families admitted into the Emergency Department, along with the healthcare team in the collaborative development of a plan for discharge and arrangement any needed post-acute care services. The position assists with gathering data to ensure that patient care provided meets approved admission criteria, continued stay, and appropriate level of care . RN ED CM I requires six (6) months of recent full-time acute care experience in an acute care hospital. RN ED CM II requires one (1) year of case management experience. Notice to Applicants: The position of RN ED Case Manager is open at two (2) tiers: I and II . Placement into a specific tier will be determined based on the candidate’s qualifications, experience, and skills. The pay for this role will correspond to the assigned tier and ranges from $52.93 to $81.58 per hour, depending on qualifications and years of experience. Requirements

  • Graduate of an accredited school of registered nursing
  • Current CA Registered Nurse (RN) License
  • RN ED CM I requires six (6) months of recent, full-time acute care experience in an acute care hospital
  • RN ED CM II requires one (1) year of case management experience

Nice To Haves

  • Bachelors of Science in Nursing (BSN)
  • Bilingual in Spanish
  • One (1) year of experience in Case Management and strong discharge planning skills
  • One (1) year of experience with InterQual
  • One (1) year of experience with Allscripts and Meditech

Responsibilities

  • Acts as a liaison between physicians, nursing, other hospital departments, community agencies, health p.ans and patients/families.
  • Responsible for discharge and transitional care planning.
  • Identifies and assures patient continuum of care needs are met.
  • Involves and educates patient/families regarding discharge planning and community resources.
  • Collaborates with appropriate community resources on behalf of the patient.
  • Refers to Social Services when appropriate.
  • Refers quality of care issues according to policy and procedure.
  • Reports suspected or actual abuse.
  • Participates in Process Improvement activities.
  • Performs other duties as assigned.
