About The Position

Job Summary: Deliver care to patients using the nursing process of assessment, planning, intervention, implementation, and evaluation. Round with physicians, implement, and follow up on orders. Collaborate with other professional disciplines to ensure effective patient care. Assist before and during dialysis procedures, following the physician's orders. Oversee dialysis technicians as they perform daily duties. Other duties as needed. Duties and Responsibilities: Supervises dialysis of chronic renal failure patients. Helps establish a plan of care for each patient and provides consistent, effective dialysis according to plan. Provides follow-up of medical orders for each patient, provides knowledge and assistance in adjusting each dialysis to each patient's particular physiologic imbalance. Assists the social worker in evaluating the social integrity of each patient and recommends interventions as needed. Sets goals (immediate and future) for the patient, planning these goals with the patient, physicians, social workers, dietitian, staff nurses, and technicians, and periodically reviews these goals with the same. Provides for safe dialysis staff checking and testing supervision of dialysis equipment and supplies before initiation of dialysis. Responsible for monitoring of patients and equipment throughout dialysis. Ensures cleaning and disinfection of equipment and stocking of supplies, and helps maintain them in a safe and efficient working order. Instruct the patient in self-care to prevent complications between dialysis treatments. Records and communicates information about patient records and plan of care to other health care personnel at the center, and transfers this information if the patient is admitted to the hospital or transferred to another outpatient center. Supervises recognition of emergencies, is knowledgeable, efficient, and provides instruction in emergency care. Makes certain emergency equipment is available and ensures immediate care to correct emergency conditions. Involves the patient in his/her care by providing education on dialysis and related topics. Includes teaching about dietary and fluid management, medications, chronic renal failure, and the role of dialysis, principles of dialysis, vascular access, and its maintenance. Coordinates consultation with dietitian. Assists in plans for unit changes. Provides for ordering of medications, supplies, and ensures that the unit is well stocked with all necessary equipment. Attends staff meeting at least 80% of the time or goes over minutes with the dialysis manager. Completes all dialysis and UBH required education. Professional Requirements: Ensures that appearance and personal conduct are professional at all times. Excellent attendance record. Wears appropriate clothing for job functions. Wears an ID badge. Works at maintaining a good rapport and a cooperative working relationship with physicians, administration, and staff. Represents the organization positively and professionally in the community. Maintains patient confidentiality at all times. Complies with all organizational policies regarding ethical business practices. Is familiar with and follows UBH Human Resource policies.

Requirements

  • Current state Registered Nurse Licensure, within the state of Utah.
  • BLS
  • Ensures that appearance and personal conduct are professional at all times.
  • Excellent attendance record.
  • Wears appropriate clothing for job functions.
  • Wears an ID badge.
  • Works at maintaining a good rapport and a cooperative working relationship with physicians, administration, and staff.
  • Represents the organization positively and professionally in the community.
  • Maintains patient confidentiality at all times.
  • Complies with all organizational policies regarding ethical business practices.
  • Is familiar with and follows UBH Human Resource policies.

Nice To Haves

  • Six months of experience in a chronic dialysis facility preferred.
