The Diabetes Educator is involved with assessment, diagnosis, educating and proactively manage the patient's condition. This includes providing one-on-one and group education sessions, developing personalized care plans, monitoring patient progress, and collaborating with other healthcare professionals. They also play a crucial role in staying updated on the latest diabetes management techniques and participating in community outreach. This position will be at the Pediatric Clinic with every 7th weekend call rotation at the Medical Center. - Monday - Friday 8am-5pm + 7th weekend Call - Experience is credit to the wage range - Must have a bachelor’s degree in nursing or willing to obtain within three years. Requirements

  • Must have a bachelor’s degree in nursing or willing to obtain within three years.
  • Graduate from an accredited nursing program preferred.
  • Two years of experience as a clinical nurse is required with demonstrated leadership skills.
  • Requires current licensure as a Registered Nurse in state(s) of practice and/or possess multi-state licensure privileges as required by position.
  • Must hold certification as a Certified Diabetes Educator from the National Certification Board of Diabetes Educators or obtain that certification within three years when eligible.
  • Basic Life Support (BLS) certification is required.

Nice To Haves

  • Previous experience and knowledge of Diabetes Management preferred.

Responsibilities

  • Educates patients on self-care involved in the diagnosis of diabetes.
  • Collects patient data to monitor health status and proactively manage the patient's condition.
  • Organizes diabetes self-care materials including nutrition, medication, physical activity and glucose testing.
  • Demonstrates familiarity with pathophysiology, epidemiology, and clinical practice consistent with practice level.
  • Makes suggestions on how to improve health conditions based on the analysis of patient data; refers patients with specific problems to other healthcare providers.
  • Applies principles of program and/or business management to create a climate that supports successful self-management of diabetes.
