About The Position

Under the supervision of the Program Administrator of the Crisis Center and the Director of Nursing, this position is part of a multidisciplinary team that provides psychosocial rehabilitative services to adult individuals with severe and persistent mental illness, acute psychiatric crisis, and co-occurring psychiatric and substance use disorders (COPSD) in a 24/7 facility. This position plays a key role on Extended Observation Unit (EOU) admissions by responding to all referrals timely. Other services and coordination of services include: medication administration including psychotropic drugs and monitor for efficacy and side effects. training and supports, crisis response intervention and follow up, comprehensive nursing assessment upon admission, nursing assessments, two-hour reassessments and progress notes, Medication Administration Record (both EOU and Crisis Respite Unit) and discharge note. The Crisis Center RN provides evidenced based services to assist individuals in maintaining or improving their ability to function in the community and work towards their person centered recovery goals. Performs clinic-based services to promote service excellence and continuous quality improvement by providing medical oversight and education to individuals served at the Crisis Center. All services provided follow federal, state, and local guidelines and are performed based in a trauma informed, person oriented, and culturally and linguistically appropriate model of care. Other duties as assigned to meet program and individual needs. Expectations include timely and thorough documentation in electronic medical record, time management and organizational skills, collaboration of treatment provided to individuals at the Crisis Center, strong teamwork abilities, and excellent customer service. Specific Targets: All Crisis Center internal and Xferall referrals and subsequent follow up are responded to within 30 minutes. Individuals admitted to the EOU receive a nursing assessment within 15 minutes of admission. All individuals on the EOU receive a nursing assessment and two-hour reassessment and progress documented each shift. No medication errors. A discharge note is complete for all individuals on the EOU. Medication room procedures including controlled substance inventory and log, medication fridge temperature checks, medication storage and disposal are followed 100% of the time. All of services notes are completed and signed by end of shift. 90% of full chart reviews receive a score of 80 or higher in following the Golden Thread. All incident reports are completed appropriately and timely. All required trainings are completed prior to due date. Attends monthly team meeting. Due to the Crisis Center operating 24/7, employees may be required to work on center holidays if the holiday falls on their scheduled workday. In the event of unforeseen circumstances that require the use of Paid Time Off, employees are expected to notify their supervisor as soon as possible but no later than 4 hours of their regularly scheduled work time. This position requires working three 12-hour shifts per week. Additional hours, not to exceed 40 hours per week, are to cover shift change, timely documentation and corrections, and to complete required trainings. If Bilingual and receiving stipend, employee will be available to translate for other staff as needed and will utilize approved language in the provision of services as needed.

Requirements

  • Graduate of a school of professional nursing approved by the Texas Board of Nursing.
  • Current, full, active, and unrestricted Texas nursing license as Registered Nurse required.
  • Must have reliable transportation and a valid Texas Driver License with a good driving record.
  • Utilize the nursing process while adhering to behavioral health policies and procedures to assess, plan, implement, and evaluate care of individuals served.
  • Possess in-depth knowledge and understanding of the psychopathology and psychotropic medications.
  • Working knowledge of Behavioral Health medications (antipsychotics, antidepressants, psychotropic agents, and mood stabilizers, and anticonvulsants)
  • Act as an advocate to individuals served and provide care in a non-judgmental, non-discriminatory manner that is sensitive to diversity while preserving their autonomy, dignity, and rights.
  • Demonstrated ability to foster collaborative relationships with various levels of professions and programs.
  • Provide/document individualized patient/family education as indicated.
  • Self-starter who works closely with all team members and takes initiative in completing tasks.
  • Knowledge of community resources, of case management principles, objectives, standards, methods, and of program policies and procedures.
  • Be able to complete detailed, yet concise progress notes on all services provided during the sessions
  • Ability to assess individual needs, to coordinate services, to communicate effectively and to train others.
  • Have the knowledge of the social services system and the ability to connect clients with appropriate services
  • Ability to write clearly and professionally
  • Have the ability to think clearly and logically, in order to deal with potentially dangerous and/or emotional situations.

Nice To Haves

  • Experience working in a community mental health clinic or CCBHC preferred.
  • Experience with electronic medical records.
  • Prefer to have at least one year of Mental Health Nursing experience.
  • Two years of inpatient nursing is preferred.
