About The Position

Requirements

  • Bachelor of Science in Nursing (BSN) from an accredited institution
  • Current Registered Nurse (RN) license in the state of practice

  • Proficiency in electronic health records (EHR) systems
  • Ability to work collaboratively with multidisciplinary teams

Nice To Haves

  • Previous experience in a coordinator or supervisory role preferred
  • Experience working in community health or outpatient settings is an advantage

Responsibilities

  • Coordinate and oversee patient care activities to ensure efficient and effective service delivery.
  • Collaborate with healthcare teams to develop, implement, and evaluate patient care plans.
  • Monitor patient progress and adjust care plans as necessary to meet patient needs.
  • Ensure compliance with healthcare regulations and standards in all patient care activities.
  • Serve as a liaison between patients, families, and healthcare providers to facilitate communication and understanding.
  • Provide guidance and support to nursing staff and other healthcare professionals.
  • Participate in quality improvement initiatives to enhance patient care outcomes.
  • Maintain accurate and up-to-date patient records and documentation.
  • Assist in the development and implementation of policies and procedures related to patient care coordination.
