About The Position

Find your purpose as a Registered Nurse Coordinator for Convenience Care! In this role you will provide nursing care for patients referred to or being seen for care in the Convenience Care clinics. This position acts as a section clinical lead, patient advocate, and care coordinator. As the RN Coordinator you will participate in coordination of patient care, department leadership, triage and provide prompt and accurate communication to the patients, physicians, and the health care team. You will maintain professional competencies by attending educational programs, participate in professional organizations, subscribe to the reading professional journals and other appropriate activities. Schedule: Full-time 80 hours every 2 weeks Monday-Friday 8am-4:30pm Must be willing to work some flexible shifts as needed Ex. 1pm-9:30pm or 7am-3:30pm

Requirements

  • Associated Degree in Nursing required.
  • 3 years’ clinical experience in ambulatory or acute care medical surgical setting required.
  • Current license as a Registered Nurse in the state of MN.
  • Basic Life Support (BLS) through American Heart Association.
  • Experience with patient education.
  • Effective communication skills.

Nice To Haves

  • Experience in surgery preferred including telephone triage, symptom management, and wound care.

Responsibilities

  • Understands and maintains knowledge of convenience care practices including Urgent Care and Occupational Health.
  • Possesses clinical assessment skills relative to outpatient acute urgent care and same day services. Coordinates information for patient care, including assessment of and education for patients being seen.
  • Identifies and maintains standard practices for urgent care and same day services relating to nursing, support, and provider practice
  • Communicates verbal and written information in a clear, concise, accurate, and timely manner. Effectively communicates with all members of the health care team.
  • Serves as a department’s lead and primary contact for patients, staff, and providers.
  • Works with providers, nursing staff, and other areas as appropriate to determine proper management of patient conditions and needs.
  • c. Promotes effective, efficient, and appropriate resource utilization to achieve desired financial outcomes.
  • Participates in improving the quality of patient care by identifying areas for improvement within the department. Communicates/collaborates with leadership on quality outcomes. Assists in the coordination, implementation, evaluation, and modification of the overall plan for care through an interdisciplinary and collaborative team process to ensure timely, comprehensive coordination of services.
  • Collaborate in the development of treatment plans and triage processes to meet patient needs and optimize disease prognosis/outcomes.
  • Advocate for the patient’s right to autonomy through shared decision making. Assist patients and health care providers to plan, develop, and obtain services to meet the needs of patients.
