About The Position

Tuesdays and Wednesdays in an office setting with flexibility of working from home/office the third day. This position provides telephonic support to our patients over a long period of time. The RN Care Resource Coordinator functions as an advocate for the patient and assists patients to reach optimal function and health in an environment that will support their needs. The continuum of care includes inpatient to outpatient to community. The RN Care Resource Coordinator applies the nursing process of assessment planning, implementation, monitoring evaluation and coordination of care to meet the patients' health care needs across the continuum in collaboration with the primary care team to ensure safe, quality and cost effective care.

Requirements

  • Bachelor's degree in nursing is required.
  • Master's degree in nursing is preferred.
  • Must have a Georgia Registered Professional Nurse license.
  • Three or more years of experience in clinical practice in an ambulatory setting or equivalent combination of education and experience in a hospital inpatient, outpatient, behavioral health or social service environment is required.
  • Experience in motivational interviewing/health coaching/goal setting is required.
  • Certification in Care Coordination or willingness to complete within 1 year is required.
  • Basic Life Support (BLS) from American Heart Association is required.
