Job Description: RN Complex Care Manager – Pediatric Population Who We Are: Southern New Hampshire Health has been a cornerstone of the region since 1893, delivering high-quality, compassionate care close to home. Our medical staff includes over 500 providers from Foundation Medical Partners, and local practices. Foundation Medical Partners, our multi-specialty group, spans 70+ practices across southern New Hampshire and northern Massachusetts, providing coordinated, patient-centered care to thousands each year. Position Overview: The RN Complex Care Manager supports children and adolescents with complex medical and social needs by coordinating comprehensive, family-centered care across the continuum. This role emphasizes partnership with pediatric patients, families, primary care providers, and community resources to address health disparities, improve outcomes, and promote self-management.

Requirements

  • Current NH RN license (BSN preferred).
  • Minimum 5 years of nursing experience in acute, community health, or ambulatory settings; pediatric experience strongly preferred.
  • Knowledge of care coordination, health coaching, and chronic disease management in children.
  • Strong communication, organizational, and family engagement skills.

Responsibilities

  • Manage a panel of pediatric patients requiring transitional and complex care management.
  • Assess health status, social determinants, and health literacy; develop individualized care plans in collaboration with families and multidisciplinary teams.
  • Provide education and coaching for chronic disease self-management, medications, and advanced care planning tailored to pediatric needs.
  • Facilitate smooth transitions between inpatient, outpatient, specialty, and community settings.
  • Engage families in shared decision-making and connect them to appropriate social and community resources.
  • Serve as an advocate and primary point of contact for patients and caregivers.
  • Monitor progress toward goals, adjust care plans as needed, and ensure timely communication across providers.
