We are seeking a Registered Nurse (RN)to join our team at our program in Westmoreland County. Earn $40.70 per hour with full benefits or $42.70 per hour with the enhanced pay option. Includes mileage reimbursement. Location: 133 Donohoe Rd Suite 103 Greensburg, PA 15601 Position Details The Registered Nurse plays a pivotal role in our Assertive Community Treatment (ACT) team, supporting rehabilitation, treatment, and crisis intervention under clinical supervision. Responsibilities encompass delivering person-centered services, coordinating treatment activities, providing supportive interventions, and facilitating skill development. As part of the ACT team, the Registered Nurse assists psychiatrists in administrative tasks, ensures the flow of medical information, and provides medication education and management to clients. The ACT team is a multidisciplinary group dedicated to supporting adults with serious mental illness (SMI) through community-based services, including therapy, medication management, and resource linkage. Services are primarily delivered in the community, with clients receiving at least three home visits per week. The RN contributes to a holistic approach to care, helping clients integrate into society while ensuring their basic needs and treatment goals are met. Frequent travel (50-90%) is required, and personal vehicle use for client transportation may be necessary.

Requirements

  • Graduate of an accredited nursing school and possess active authorization to practice as a Registered Nurse in Pennsylvania.
  • 2 years of experience working with individuals with mental illness
  • Valid PA driver's license. Travel is necessary to offer support across Westmoreland County.

Responsibilities

  • Community-Based Client Support (50-90% Travel). Provide direct care in clients' homes and community settings, conducting health assessments, monitoring medication adherence, and addressing medical concerns.
  • Oversee the medication system, document treatments, educate clients and team members, and ensure compliance with prescribed regimens.
  • Work closely with medical providers, behavioral health professionals, and community resources to ensure integrated, comprehensive client care.
  • Participate in crisis response, support treatment planning, and provide health and wellness education to clients, their supports, and the ACT team.
