About The Position

The RN Community Educator serves as the clinical and community representative for Commonwise Home Care in the Williamsburg market. This role blends nursing expertise, community outreach, Caregiver support, and operational collaboration to ensure high-quality care delivery and support local growth. The RN Community Educator works closely with clients, families, caregivers, healthcare partners, and internal teams to uphold Commonwise’s care standards and values.

Requirements

  • Active RN license in the state of Virginia
  • Minimum of 5 years of nursing experience
  • Strong communication and relationship-building skills
  • Ability to work independently and manage multiple priorities
  • Comfort working in community-based and home settings

Responsibilities

  • Clinical Care Management
  • Conduct comprehensive in-home client assessments
  • Develop, implement, and update individualized care plans
  • Monitor ongoing care and adjust plans as client's needs change
  • Provide clinical guidance and support during care transitions or urgent situations
  • Coordinate care with clients, families, Caregivers, and healthcare providers
  • Community Outreach
  • Represent Commonwise Home Care within the Williamsburg community
  • Build and maintain relationships with referral partners and community organizations
  • Participate in local events, meetings, and educational activities
  • Collaborate with internal teams to support local growth and referral efforts
  • Caregiver Support and Development
  • Assist with caregiver recruitment and onboarding efforts as needed
  • Provide clinical training, mentorship, and ongoing support to Caregivers
  • Conduct performance check-ins to ensure care quality and adherence to standards
  • Client and Caregiver Coordination
  • Collaborate with the Care Coordination team to align clients and Caregivers effectively
  • Maintain regular communication with clients and families to assess satisfaction
  • Address concerns and support positive care experiences
  • Operational Collaboration
  • Partner with internal teams to support care delivery and operational needs
  • Serve as the local clinical resource for the Williamsburg area
  • Participate in team meetings and contribute to process improvement efforts
