The RN Community Clinical/Resource Nurse is a member of an interdisciplinary team of healthcare workers who is responsible for coordinating community events to provide education and counseling on healthy lifestyles and behaviors. This nurse collaborates with consumers in community, corporate, and congregational settings to provide high quality evidenced based information and education on pertinent health topics. The framework for practice is steered by the WellStar Professional Practice Model, 5-Star Nursing and evidence-based practice and research. RN competencies are derived from these models and supported by the WellStar Core Competencies. It is expected that the Registered Nurse is licensed, knowledgeable and upholds the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of Practice and Code of Ethics Standards put forth by the American Nurses Association. As a member of the Community Health Education & Outreach team, it is expected that the individual represents the needs of the community and upholds system and departmental policies and procedures while supporting service excellence goals.

Requirements

  • Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred
  • RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
  • BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
  • Minimum 1 year nursing practice that includes sufficient clinical and process management experience to insure competence.
  • Strong technical and clinical skills needed to perform a variety of screenings that may include blood pressure, fingerstick, blood draw, etc
  • Must possess ability to act as a team player and handle fast-paced work environment.
  • Exercises independent judgement and initiative when performing duties.
  • Must be strongly motivated for community outreach in working with people.
  • Must possess personal commitment to healthy behaviors and a healthy lifestyle.
  • Knowledge of various health topics needed for patient education.

Responsibilities

  • Performs the Nursing Process (assessment; diagnosis; identification of outcomes; planning; implementation and evaluation) where applicable, in the performance of community outreach using evidence base practice, analytical/critical thinking to assist the community through effective partnerships recognizing that caring is central to achieve optimal patient care outcomes.
  • Coordinates the delivery and appropriate documentation of educational programs delivered to community residents that promotes the professional care delivery model.
  • Practices using current clinical practice standards.
  • Performs basic health screenings such as blood pressure (BP) monitoring and/or vaccinations in areas of competency as designed by the Community Benefit department and aligned with the Center of Equity strategic domains.
  • Educates the community on health prevention, maintenance, and promotion strategies to facilitate positive health outcomes.
  • Assist with specific health screening test such as checking BP and/or vaccinations at community events.
  • Always maintains confidentiality and professionalism.
  • Refers patients to appropriate healthcare provider when necessary.
  • Demonstrates teamwork and collaboration by practicing in a manner that is fiscally responsible and promotes the recruitment and retention of all healthcare team members.
  • Assist with staffing Community Benefit events on a consistent basis.
  • Delivers community outreach programs according to the Center for Health Equitys strategic domains.
  • Maintains consistent work schedule according to policy.
  • Adheres to departments dress code policy.
  • Serves as event team leader when assigned which includes serving as the point person for the external site coordinator and the Wellstar PRN team for the duration of the event; troubleshooting any issues or incidents; and following up as appropriate with supervisor or designee.
  • Participates and supports performance improvement inclusive of all stakeholders, research, and research utilization to promote quality outcomes, including initiating and/or leading such activities as well as, promoting an inter/intra-disciplinary process.
  • Performs duties as assigned with the Scope of Nursing Practice and supported by Wellstar Health System.
  • Completes all initial and ongoing professional competency assessments, required mandatory education, and population specific education.
  • Maintains current certifications, including CPR for Healthcare Provider and all required credentials/licensure.
  • Serves as a preceptor and/or mentor for other professional nurses and other staff members as appropriate to ensure that there is a current and future qualified workforce; modeling the professional practice of nursing and creating a healthy work environment.
  • Promotes evidence-based practice in the delivery of community education and outreach programs.
  • Upholds all healthcare system/organizational policies and procedures and clinical competencies put forth by this job description and professional association including legal, regulatory and accreditation requirements and standards ensuring by way of example, such goals as The Joint Commission Patient Safety Goals and Wellstar Health Systems safety absolutes.
  • Follows established policies, procedures, guidelines, and curriculum developed by Wellstar Community Benefit when performing duties.
  • Reviews department handbook and any revisions when necessary.
  • Participates in data collection as necessary to advance evidence-based practice when appropriate. Consults appropriate experts and uses appropriate resources and evidence to address practice questions.
  • Participates in the development of protocols and procedures when called upon or through self- initiation and in collaboration with manager and other members of the healthcare team to achieve best practice outcomes (i.e., increased health promotion; awareness and risk assessment of chronic disease; team huddles and event feedback; etc.).
  • Supports efficient and effective use of human and material resources.
  • Packs and transports supplies to community events as needed.
  • Proactively plans for community events both immediate and ongoing.
  • Assist team members in the proper utilization of department resources/supplies.
  • Performs other duties as assigned
  • Complies with all Wellstar Health System policies, standards of work, and code of conduct.
