Support people with Intellectual and Developmental Disabilities (IDD) in their homes to achieve maximum independence. Provide reliable, compassionate and consistent care while creating quality-based relationships in a supportive environment.

  • Monitor, assess and advise the team on health needs, while promoting health and wellness through nursing processes.
  • Create a positive, responsible and open communication style with everyone supported including all employees, family members and other health care providers.
  • Outline all monthly appointments; provide documentation and information needed to successfully complete the appointment.
  • Attend appointments, as needed.
  • Clarify hospital discharge, interagencies, medication orders, and/or plans of care when needed.
  • Maintain records including: monthly nursing reports, plans of care, monthly medication charts and book, incident reports, medical history, hospital intake/discharge, immunization history, nursing review.
  • Participate in hospital discharge or intake process.
  • Provide instructions and education to those supported and staff members as it relates to medical diagnoses and healthcare needs.
  • Coordinate care with clinics, physicians and hospitals.
  • Provide training to team members including: competencies, medical background, treatments and wellness plans.
  • Review medicine closet periodically for supplies.
  • Promote resident empowerment and self-care activities including self-medication.
  • Provide guidance and support to staff designated to order and stock medications.
  • Provide coverage for vacancies and peers during vacations.
  • Assume responsibility for ongoing professional development by participating in community-based educational trainings.
  • Perform all other duties as assigned by supervisor(s).
  • Graduate from an accredited school of nursing with a Bachelor's of Science in Nursing.
  • Experience preferred providing care for chronic, acute and mental health needs.
  • Must possess a valid Rhode Island Nursing license.
