About The Position

The Clinical Team Manager at Elara Caring plays a crucial role in overseeing patient care programs, ensuring clinical and administrative outcomes, and providing professional support to clients. Requirements

• Associate degree in science or a bachelor's degree from a School of Nursing is preferred.
• 5 years of experience as a registered nurse in a Home Health service or Hospice environment, preferably in a leadership role.
• Current State License as a Registered Nurse.
• Valid driver's license and insurance, along with reliable transportation.

Responsibilities

• Oversee the implementation and supervision of all patient care programs.
• Ensure achievement of clinical and administrative outcomes.
• Coordinate care during the hospice benefit period.
• Engage and support staff to ensure their wellbeing and safety.
