About The Position

According to the prescribed policies and procedures of the organization including all applicable federal, state and local regulations and reporting to the Director, the Clinical Operations Coordinator is responsible for ensuring smooth daily operations and enhancing patient care within the home health agency. Key responsibilities include overseeing operational tasks such as order approval, utilization management, regulatory compliance, documentation auditing, staff education, and offering support to staff in troubleshooting operational issues.

Requirements

  • Minimum of an associate’s degree in nursing field (RN) as a registered nurse from an approved school of nursing
  • A minimum of 2 years home health leadership experience or equivalent experience in a clinical or operational role.
  • Must be able to demonstrate a strong understanding of home health regulations, conditions of participation, reimbursement and industry best practices.
  • Must have excellent communication, comprehension, and interpersonal skills.
  • Must have the ability to speak, read, write and understand English, as well as the ability to make self- understood.
  • Must have strong working computer skills with basic professional computer software (Microsoft Office suite, Windows, etc.) and the capacity for learning new software systems quickly.
  • Must be self-directed with excellent organizational skills and the ability to manage tasks and assignments concurrently.
  • Must be able to successfully lead others in an environment of complexity.
  • Must have strong motivational skills and a high emotional intelligence quotient (EQ).
  • Sometimes, up to 25%. Must have a valid driver’s license, automobile insurance and qualified to drive under the organization’s motor vehicle check.
  • Must hold an active Registered Nurse (RN) license in the state of practice.

Nice To Haves

  • Home health certification (e.g., CHPN, CCM, or equivalent) is preferred.

Responsibilities

  • Quality Improvement and Data Integrity: Participate and drive in quality improvement initiatives, helping to address trends affecting agency outcomes and processes. Review visit utilization and patient conditions to ensure care guidelines are met. Collaborate with clinicians to resolve any data integrity issues and ensure corrections are made according to organizational policies.
  • Clinical Care Oversight and Coordination: Collaborate with the interdisciplinary team to ensure proper care plan development, reviews, and ongoing patient assessments. Formulate and approve care plans, progress notes, and interventions while monitoring patient outcomes and identifying areas for improvement in care delivery.
  • Regulatory Compliance: Stay current on home health regulations, reimbursement guidelines, and industry standards. Ensure all clinical actions comply with relevant rules, regulations, and conditions of participation.
  • Incident Management and On-Call Support: Investigate and resolve patient, family, physician, or staff concerns in a timely manner. Participate in on-call rotations as needed, ensuring continuous support for clinical operations.
  • Physician and Staff Communication: Maintain clear communication with physicians to follow up on orders, resolve issues, and ensure timely receipt of required documentation. Provide ongoing support and troubleshooting to field staff, helping them navigate clinical and operational challenges. Assist in the training and education of field staff on home health regulations, documentation standards, and patient care processes.
  • Supervision and Staff Development: Support or directly supervise nursing staff, including conducting interviews, orienting new hires, managing schedules, and overseeing performance evaluations and corrective actions. Provide guidance on patient care issues and identify ongoing educational needs for staff.
  • Utilization and Compliance Management: Oversee utilization management, ensuring appropriate care is provided by reviewing and managing new admissions (SOCs) and recertifications, working closely with field teams to optimize scheduling and resource utilization. Conduct regular documentation audits to ensure compliance with regulatory requirements and quality standards.
  • Customer Service and Problem Resolution: Deliver exceptional customer service to clients, families, staff, and other stakeholders. Address inquiries and concerns promptly, investigate complaints, and provide feedback to Directors of Clinical Management and the Regional Director of Operations.
  • Operational and Workflow Support: Assist the Directors of Clinical Management with daily operations, ensuring workflows are completed efficiently and in compliance with company policies. Manage tasks in the Home Care Home Base (HCHB) EMR system, ensuring accurate documentation and progress of all activities.
  • Team Collaboration and Professional Development: Foster a professional atmosphere within the agency, attending monthly staff meetings, nursing in-services, and participating in ongoing educational activities to maintain knowledge and skills in home health care.
