About The Position

Are you an experienced Registered Nurse (RN) ready to lead in home healthcare? Boost Home Healthcare is seeking a Clinical Manager to oversee and coordinate high-quality care for homebound patients. You’ll supervise clinical operations, ensure compliance with care plans, and guide an exceptional team of clinicians.

Requirements

Licensed RN in MA with 2 years of nursing and 2 years of home health experience
1 to 2 years of experience supervising clinicians
Proficient in OASIS, EMR, Medicare CoPs, and QAPI
Strong communication, leadership, and organizational skills
Strong organizational, planning, problem solving, decision-making, and leadership skills
Valid driver’s license, CPR certification

Responsibilities

Lead clinical team and manage patient care coordination
Oversee OASIS documentation, PDGM coding, and patient care planning
Monitor case compliance and participate in QAPI activities
Support staff hiring, development, and performance reviews
Occasionally provide direct care in urgent situations
Monitor staff assignments, caseloads, and team productivity. Promote efficient utilization of resources to achieve desired clinical outcomes and within episode reimbursement.
Provide the Administrator with census, staffing, team productivity and other reports as requested. Identify staffing patterns and needs of the assigned team.
