Clinical Manager (RN) Driving Excellence in Patient Care Coordination . As the Home Health Clinical Manager , you will serve as a key member of our leadership team, responsible for overseeing the coordination, quality, and delivery of clinical services. You will guide care teams, ensuring compliance with care standards, and support clinicians in providing safe, effective, and compassionate care. You will act as a resource for staff, facilitate communication among providers, and help shape care plans that meet the unique needs of each patient. Your leadership will directly influence patient outcomes and the overall success of our care programs.

Requirements

  • Minimum of 2 years' experience managing a team
  • Must be licensed as a Registered Nurse (RN) in the State of Arizona
  • Proven supervisory or nurse management experience leading clinical teams in home health OR critical care experience including ICU or emergency medicine

Responsibilities

  • Oversee coordination and management of patient care plans
  • Ensure proper development, implementation, and updates of individualized plans of care
  • Communicate with physicians and integrate orders into care plans
  • Review patient information and assign appropriate clinical personnel
  • Guide and support clinicians to promote effective performance and quality care
  • Conduct case conferences and participate in interdisciplinary team meetings
  • Assist with staff screening, interviewing, orientation, and education programs
  • Participate in quality assessment and performance improvement initiatives
  • Maintain compliance with HIPAA privacy laws while managing sensitive medical records and documentation processes
  • Promote a positive work environment that encourages teamwork, professional growth, and high morale among nursing staff
