Requirements

Active, unrestricted RN license in the state of practice.
Current CPR certification.

Nice To Haves

Minimum of two years RN experience (preferred)
At least one year of Home Health experience (preferred)

Responsibilities

Provide daily clinical oversight of patient care services.
Coordinate referrals and admissions with internal and external sources.
Evaluate patient needs and agency capabilities before admission.
Supervise care planning, partner assignments, and care delivery.
Ensure compliance with regulatory standards and agency policies.
Mentor and coach clinical staff to promote morale and retention.
Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.
