About The Position
The Clinical Manager coordinates and oversees all direct care patient services provided by clinical personnel. This role involves developing, planning, implementing, analyzing, and organizing clinical operations for a specific location managed. The Clinical Manager conducts and delegates the assessment and reassessment of patients, including updating care plans and interpreting patient needs, while adhering to company, physician, and/or health facility procedures and policies. They manage the assignment of caregivers and are responsible for overseeing the delivery of care to all patients served by the location. The Clinical Manager receives case referrals, reviews available patient information related to the case, and determines home health or hospice needs. They are accountable for ensuring patients meet admission criteria and make decisions to admit patients to service, assigning appropriate clinicians as needed. Additionally, the Clinical Manager instructs and guides clinicians to promote effective performance and delivery of quality home care services, assists in establishing therapeutic goals, monitors cases for compliance with regulatory agencies, coordinates communication between team members, and participates in sales and marketing initiatives. They also supervise clinical employees, handle employee corrective actions, participate in hiring and training, assess staff education needs, and ensure adherence to internal policies and standards. The role includes analyzing situations, identifying problems, and performing QA/PI activities, while also providing direct patient care in emergencies and acting as Branch Director in their absence.
