Responsible for building relationships in the contracted healthcare facilities. Identifies potential referrals and facilitates timely admissions in the nursing facility. Responsible for the education and training of healthcare facility staff, especially in the area of hospice care and pain and symptom management. Assures that appropriate documentation which reflects collaboration and coordination of care is available in the nursing facilities’ medical record. Provides end of life education to healthcare professionals as well as the public. Explains hospice benefits to patients and families and obtains consent for hospice services, working with the admissions’ department.
