The Home Care Registered Nurse is an experienced nurse who provides hands on care to medically complex patients with a wide variety of diagnosis. Our interdisciplinary care teams work together to provide exceptional care to patients. The Registered Nurse serves as the leader of the health care team establishing a relationship with the patient and their caregivers, collaborating with physicians and other team members to assess, plan, implement, and evaluate an individualized plan of care that promotes optimal health.   • Provide hands-on care to medically complex patients with a wide variety of diagnoses.
  • Lead the health care team in establishing relationships with patients and caregivers.
  • Collaborate with physicians and team members to assess, plan, implement, and evaluate individualized care plans.
  • Integrate ANA Nursing: Scope and Standards of Practice and the ANA Code of Ethics in daily practice.
  • Support onboarding of new team members and precept as applicable.
  • Engage in performance improvement activities and adhere to clinical documentation guidelines.
  • Graduate of an accredited nursing program required.
  • Bachelor of Science in Nursing required within 5 years of hire.
  • 1 year of relevant experience.
  • Licensed to practice as a Registered Nurse in the Commonwealth of Virginia required.
  • Valid Virginia Driver's License required.
  • American Heart Association (AHA) Health Care Provider BLS certification required.
