About The Position

The Clinical Expediter is a pivotal role focused on improving patient access and flow, and enhancing the quality of care through clinical expertise, leadership, organizational awareness, and critical thinking. This position primarily addresses the latter half of the care progression, concentrating on resolving delays in care and discharge barriers. The Expediter will take patient-specific actions to improve patient experience and flow. They will also collaborate closely with CareComm leadership to prioritize daily tasks, such as identifying the highest need for vascular beds. The role involves utilizing real-time and predictive information within CareComm to monitor, triage, and address alerts on relevant tiles like 'Delays in Care', 'Discharge Planning', and 'Patient Pathways'. The Clinical Expediter will investigate these alerts using the electronic medical record or by contacting appropriate parties to determine a resolution. Once a resolution path is decided, they will communicate and partner with the local care team to support its implementation.

Requirements

  • Licensure to practice as a Registered Nurse in the State of Florida
  • Registration with related professional association
  • Five (5) years of experience in related area of practice
  • Ability to innovate, problem solve, build relationships, and negotiate

Responsibilities

  • Improve patient access/flow and enhance quality of care using clinical expertise, leadership, organizational awareness, and critical thinking skills
  • Focus on resolving delays in care and discharge barriers in the latter half of the care progression spectrum
  • Focus on actions that are patient specific to improve patient experience and flow
  • Work closely with CareComm leadership to prioritize work for the day (e.g. highest need is vascular beds)
  • Utilize real-time and predictive information in CareComm
  • Monitor, triage, and address alerts on relevant tiles (ex: Delays in Care, Discharge Planning, Patient Pathways)
  • Investigate alerts using the electronic medical record and/or contacting the appropriate parties
  • Determine the appropriate action for resolution
  • Communicate / work in partnership with the local care team to support resolution
