This position can expect to lead system-wide strategies that grow and deepen the impact of evidence-based health promotion programs. They are responsible for operational efficiencies and outcomes of these educational programs. This position will lead strategies for growing relationships with Sanford leaders, instructors, care team members, and community partners throughout the Sanford footprint. They are expected to demonstrate leadership in integrating chronic disease self-management education within emerging care models and expanding a core set of programming. Opportunity to make a difference in the lives of those living with physical and/or mental health conditions via educational programming. Ability to work remote with occasional in person meetings at community sites, clinics, other office settings, etc. Assist with other projects related to patient and community education.
