About The Position

The Clinical Manager at SAFREST Resources, LLC will oversee clinical operations within a rapidly growing home healthcare agency in Philadelphia. Requirements

• Bachelor of Science in Nursing (BSN) required.
• Minimum of 4 years of experience as an RN Case Manager, Field RN, or Home Health RN.
• Proven experience with OASIS documentation and assessments for compliance with Medicare and Medicaid guidelines.
• Familiarity with the Philadelphia area, including local healthcare resources and community organizations.
• Valid driver's license and reliable vehicle for patient visits.
• Strong entrepreneurial mindset with a desire to grow a business.
• Willingness to transition from a hands-on clinical role to an administrative and leadership role.
• Excellent verbal and written communication skills.

Responsibilities

• Lead and manage the clinical team to ensure exceptional patient care and compliance with agency policies.
• Perform home visits for initial assessments, care planning, and ongoing evaluations of patients' conditions.
• Collaborate with Registered Nurses, Certified Nursing Assistants, and other healthcare professionals for seamless care coordination.
• Oversee complex patient cases to ensure personalized, comprehensive care.
• Ensure compliance with local, state, and federal regulations and industry standards in clinical practices.
• Complete OASIS assessments and ensure accurate documentation in compliance with Medicare regulations.
• Participate in business development activities to help expand the patient base.
• Transition into a more administrative and strategic leadership role as the agency grows.
