About The Position

The RN Clinical Coordinator is responsible for care coordination and supporting clinical quality across health center sites. This position plays a central role in case management functions, including hospital transfers, discharge follow-up, coordination of care with primary care providers, and monitoring of clinical quality measures. The RN Clinical Coordinator supports the standardization of clinical processes and assists with evaluating the skills and competencies of clinical staff. This position also participates in quality improvement, regulatory compliance, immunization oversight, and clinical triage as needed. The RN Clinical Coordinator reports directly to the Practice Manager at the assigned clinic site and shares supervisory responsibility of clinic Medical Assistants with the Practice Manager.

Requirements

  • A bachelor’s degree from an accredited school or college of nursing is required.
  • Licensed to practice nursing in the state of Arizona as RN is required.
  • At least three (3) years nursing experience.
  • At least two (2) years of supervisory experience required in healthcare, social services or primary care/family medicine setting.
  • Must possess a valid and unrestricted Arizona driver’s license (within 30 days of hire if applicant is from out-of-state) with no DUI/DWI or reckless driving convictions in the last five years. No more than two at-fault accidents in the last three years. Must maintain a valid Arizona driver’s license during employment. Must be insurable under NATIVE HEALTH’s liability auto policy.
  • Must pass a criminal background check and obtain a Class I Fingerprint Clearance Card within the initial ninety (90) calendar days of employment and retain a valid card while employed with NATIVE HEALTH.
  • Must pass substance abuse testing upon employment and submit to random testing during employment.
  • Must provide an updated immunization (IZ) record at the time of employment.
  • Must have a current (within 12 months) Tuberculosis (TB) skin report upon employment and provide an annual update.
  • Must obtain Basic Life Support (BLS) certification within (90) calendar days of employment and maintain certification during employment.

Responsibilities

  • Coordinate and oversee case management and care coordination activities.
  • Monitor and support clinical quality measures and continuous improvement initiatives.
  • Assist in evaluating clinical staff skills and competencies.
  • Provide oversight and support for immunization programs across multiple sites.
  • Provide clinical triage support and contribute to patient care when needed.
  • Collaborate across departments to support integrated, patient-centered care delivery.
  • Support the organization as needed.
