About The Position

Bring your clinical expertise to a role where it truly matters. As an RN Clinical Coordinator at LifeCircles PACE, you’ll provide participant-centered nursing care across clinic, home, and facility settings—using PACE-focused assessments and advanced clinical judgment to manage complex needs and support individualized care goals. Partnering with the Interdisciplinary Team, you’ll coordinate holistic, high-quality care that improves outcomes and brings the PACE model to life—helping older adults live with dignity, independence, and confidence.

Requirements

• Diploma or degree from an accredited School of Nursing or University.
• Current Registered Nurse (RN) license to practice in the State of Michigan.
• Minimum of one year of experience working with the frail elderly or long-term care population.
• Strong clinical assessment and nursing skills with the ability to manage complex medical situations.
• Ability to work independently and as part of an interdisciplinary team (IDT).
• Excellent communication and interpersonal skills, with the ability to work effectively with participants, families, and the healthcare team.

Nice To Haves

• Previous experience in home care or long-term care is preferred.
• Knowledge of PACE-specific care, interventions, and regulatory standards is a plus.
