RN Clinical Case Manager Almost Home Kids Naperville Full time, exempt (salaried) Naperville Almost Home Kids location The RN Clinical Case Manager collaborates with the interdisciplinary team in the identified plan for referral and transfer of the patient from one facility to another. The role reflects appropriate knowledge of RN scope of practice, current state requirements, CMS Conditions of Participation, EMTALA, and other Federal or State regulatory agency requirements specific to discharge planning. The primary element of the Clinical Case Manager role is care coordination, including discharge planning.

Requirements

  • Registered Nurse (RN) licensure in state of Illinois required.
  • BSN required.
  • 4+ years of relevant RN clinical experience required.
  • Current knowledge of health-care trends and practices related to pediatric health care and case management is required.
  • Experience working in partnership with health-care professionals and support staff is required.
  • Demonstrate the ability to problem solve and support teamwork in a multidisciplinary setting. Must be flexible and adapt to a changing environment.
  • Computer literacy including Microsoft Word operation and excellent written and verbal communication skills are necessary.
  • American Nurse Credentialing Center–recognized certification relevant to area of responsibility required within 12 months of hire is required.

Nice To Haves

  • Working knowledge of payer requirements, contracts, and managed care is a plus.
  • Demonstrate the ability to provide leadership to others in a collaborative and facilitative style within the Almost Home Kids model.
  • Bilingual proficiency is preferred but not necessary.

Responsibilities

  • Collect in-depth information about a patient’s situation and function to identify individual needs in order to develop a comprehensive case management plan that will address those needs.
  • Must be able to provide most information on each patient in the facility at all times (i.e., diagnosis, projected length of stay, where patient stands in the certification process).
  • Performs direct patient and family contact.
  • Determines and documents specific objectives, goals, and actions designed to meet the client’s needs as identified through the assessment process. This plan should be action oriented and time specific.
  • Executes and documents specific case management activities and/or interventions that will lead to accomplishing the goals set forth in the case management plan.
  • Determines and documents the case management plan’s effectiveness in reaching the desired outcomes and goals at appropriate intervals.
  • Modifies or changes the case management plan in its entirety or in any of its component parts, if necessary. Provides precertification and utilization review functions with insurers, and verifies funding sources for intake.
  • Obtains pertinent medical information regarding diagnosis, treatment, medical history, and current medical status.
  • Updates ongoing medical records.
  • Identifies current and potential care needs, and interfaces with child, family, patient and physician to assist in implementation and coordination of a plan to meet these needs.
  • Establishes and maintains a therapeutic relationship, and acts as an advocate and resource for the patient and family.
  • Documents all contacts regarding medical care and status, vendor/provider contacts, and information obtained clearly and concisely in the patient’s record.
