About The Position

The RN Clinical Coordinator - HUB - Per Diem position is responsible for coordinating house wide patient flow activities for all patients needing placement, unit transfers, and discharges on a 24/7 basis in conjunction with the Hub Team and all Clinical Care Coordinators. The role involves updating the logistics system when patient placement needs change, serving as a liaison between clinical departments for decisions related to patient placement, monitoring and analyzing patient throughput processes, and documenting patient placement data. The coordinator utilizes nursing knowledge to triage patient assignments based on needs, medical necessity, and bed availability, and collaborates with various teams to ensure efficient patient flow and resource allocation.

Requirements

  • Bachelor’s of Science in Nursing required.
  • Current licensure to practice nursing in the State of Illinois.
  • Two years of experience in an acute care setting required.
  • Previous leadership experience required.
  • Case management certification required or obtained within one year of eligibility.

Nice To Haves

  • General understanding of case management processes including discharge planning, health care reimbursement, utilization review process, and applicable community resources preferred.

Responsibilities

  • Coordinates house wide patient flow activities for all patients needing placement, unit transfers, and discharges.
  • Updates the logistics system when patient placement needs change.
  • Serves as a liaison between all clinical departments for decisions related to patient placement during the shift.
  • Monitors and analyzes patient throughput processes.
  • Documents, analyzes, and reports patient placement data.
  • Utilizes nursing knowledge to triage assignment of patients to appropriate target unit.
  • Establishes prompt and appropriate patient placement plans.
  • Assigns Admission Discharge Transfer Nurses to units based on demand/capacity.
  • Obtains initial patient placement orders for direct admits and transfers.
  • Works with the Hub Clinical Care Coordination Assistant to ensure pre-certification authorization is obtained.
  • Assists with prioritizing patient flow to ancillary testing and procedure areas.
  • Collaborates with the HUB Team to adjust required resources in real-time.
  • Provides hospital capacity change updates to unit and ancillary patient care teams.
  • Reviews patient admission criteria and discusses cases not meeting criteria with physicians.
  • Escalates admission criteria issues to the Medical Director of Utilization Management.
  • Manages Hub/Care Coordination facility staffing process.
