About The Position

Details Department: Primary Care Schedule: Monday - Friday 8am - 5pm The employee will travel between the following locations: 13420 N Meridian St Ste 240 Carmel IN & 13914 Southeastern Parkway Ste 108 Fishers IN 46037 Hospital: St. Vincent AMG Location: Fishers, IN

Requirements

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required. Valid state license required specific to the state in which the associate works.
  • Diploma from an accredited school/college of nursing OR required professional licensure at time of hire.

  • Patient and family communication: Clearly communicate with patients and their families about their condition, next steps, and potential treatment options.
  • Care coordination: Coordinate with providers and other members of the healthcare team, communicate patient status, and facilitate necessary referrals or follow-ups.
  • Patient education: Provide education on medical conditions, treatments, and self-care strategies to help patients manage their health.
  • Administrative tasks: Assist with various administrative duties such as managing medication refills, reviewing lab results, and scheduling appointments.
  • Valid Registered Nurse (RN) license.
  • Strong clinical assessment and critical thinking skills.
  • Proficiency in using electronic health records (EHR) and other medical software.
  • Excellent verbal and written communication skills.
  • Ability to remain calm and make quick decisions under pressure.
  • Knowledge of medical terminology, procedures, and protocols.
  • Compassionate and empathetic nature.
  • Ability to multitask and prioritize effectively.

Nice To Haves

  • Phone triage experience is preferred. This is a phone triage position for two busy Internal Medicine offices. Applicant should have at least 3 years of triage experience.

Responsibilities

  • Provide direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
  • Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate.
  • Serve as a primary coordinator of all disciplines for well-coordinated patient care.
  • Note and carry out physician and nursing orders.
  • Assess and coordinate patient's discharge planning needs with members of the healthcare team.
