About The Position

The RN Coordinator within the Avera Marshall Clinic is accountable for coordination of patient care of the primary care practice to promote effective education, self-management support and timely health care delivery.

  • Registered Nurse (RN) - Board of Nursing
  • An active license in the state of practice Upon Hire
  • 1-3 years nursing services within a clinic setting

  • Comply with safety principles, laws, regulations, and standards associated with, but not limited to, CMS, The Joint Commission, DHHS, and OSHA if applicable.

Responsibilities

  • Develops workflows to ensure smooth transition of care for patients treated within the facility, including inpatient and emergency unit.
  • Determines and completes referrals when appropriate.
  • Assists in planning short and long term business objectives in partnership with physician groups by assessment of needs and utility of the care coordination process.
  • Collaborates with providers and staff in identifying appropriate patients for care management and their progress throughout.
  • Assess barriers with patients meeting treatment goals and developing plans to overcome.
