Job Summary: Location: Methodist Professional Building Address: 8111 Dodge St. - Omaha, Ne Work Schedule: Monday through Friday, 8:30am - 5pm Position qualifies for a $2500.00 sign on bonus Collaborate with Bariatric Clinic staff to risk-stratify patients authorized for surgery for the purpose of identifying patient and/or support system characteristics that have the potential to negatively impact the success of the post-surgical recovery and clinical outcomes. Coordinates and conducts day-to-day program activities in conjunction with the program coordinator and bariatrics team. Establishes priorities and manages all clinical actives efficiently and effectively. Ensures patient satisfaction throughout all clinical and follow-up processes. Responsibilities: Essential Functions Perform primary health care for patients by utilizing the processes of assessment and diagnosis of health status. Obtain patient's medical and history information, data collection is prioritized according to immediate condition or needs. Develop and implement treatment, follow-up and evaluation plans for patient outcomes. Collaborate with physicians and other disciplines as appropriate and provides regular and consistent feedback. Use appropriate assessment techniques & diagnostic information. Performs tests relevant to patient condition and record findings. Order appropriate ancillary tests. Use new or additional data to form a treatment plan. Perform telephone triage to determine patient condition and provide appropriate consultation scheduling appointments when needed. Incorporate research into clinical practice as appropriate. Implement interventions consistent with the established plan of care in a safe and appropriate manner. Apply established therapeutic practices to patient's problem. Administer medications and immunizations using the appropriate method and with physician oversight if applicable. Generate referrals to appropriate services as needed to further diagnose and/or progress of care. Evaluate patient care to determine response to care/interventions for optimal patient outcome. Evaluate the patient's capabilities and/or limitations related to nursing and medical interventions. Recognize and address patient and family capabilities and limitations (physical and mental). Return patient telephone calls. Monitor outcomes and document patient response to interventions. Seek out new treatment methods necessary to ensure patient progress. Guides patients, families and caregivers to informed decision-making by collaborating with multidisciplinary team to support optimal patient care across the care continuum, and develops a comprehensive, proactive discharge plan for all patients. Provide patient and family teaching and education specific to the patient's health care. Proper use of medication and health/illness prevention to ensure adequate knowledge/awareness. Interview and advise specific to patient needs. Demonstrate sensitivity to family regarding need for patient information on a timely basis. Work collaboratively with physicians and other team members to provide appropriate patient education geared to the patient's needs and family's ability to provide care. Use adult learning principles for teaching purposes being careful to check for understanding and being available for follow-up questions. Recommend references and resources such as printed materials, online, community and other information sources to fulfill the family's need for education. Maintain a professional behavior in attitudes, demeanor, and interactions to achieve organization's mission. Communicate effectively with physicians, staff and patients. Is approachable and professional in working with other health professionals, patients and their family. Maintain a professional appearance, behavior, and work environment. Initiate action to correct, prevent, and/or minimize risk(s) to the patient, family or hospital/clinic employees (i.e., falls, needle sticks, patient concerns). Communicate to appropriate personnel regarding unresolved issues. Serve as a role model in the provision of emerging care. Serve as a reliable/knowledgeable resource for staff. Participate in meetings, research on special projects and professional organizations. Maintain complete patient records. Document data appropriately. Document treatment plan of care that is individualized, appropriate and reflects patient's needs. Document interventions. Document consultations with other practitioners/physicians. Participate in performance improvement and outcome management. Meet regularly with medical director and program coordinator for chart reviews. Identify concerns in patient care & follow up according to hospital policy. Participate in patient satisfaction and other quality improvement initiatives. Participate in tracking of variances in care. As the RN Bariatric Coordinator, demonstrates the following: Collaborates on an ongoing basis with Medical Director, program coordinator, clinic staff, and physicians to disseminate information and promote positive communication techniques interdepartmentally. Works with Medical Director and program coordinator to anticipate and identify the needs of the center and creatively implement solutions to problems. Integrates Evidence Based Practice into clinic improvements. Works in conjunction with Bariatric Surgery Clinic and hospital clinical leadership in developing orientation and educational programs for staff regarding the special needs of bariatric patients, such as: sensitivity training, lift and transfer techniques, complications of bariatric surgery, and post-op psycho-social needs. Identifies patient and/or support system characteristics that have the potential to negatively impact the success of the post-surgical recovery and clinical outcomes. Provides on-site coordination to the bariatric clinic by collaborating with the program coordinator and Bariatric physicians to risk-stratify patients authorized for surgery. Communicates to team members the risk stratification information to be addressed during pre-surgery, surgery, acute care, and post-discharge phase. Proactively and effectively communicate patient/family needs to interdisciplinary team and facilitates timely involvement of other health care team members (i.e.: Social Work, Wound Care, Diabetes Educators, Dieticians) to assure priority patient needs are met and appropriate post-discharge services are established. Coordinates communication between multiple physicians (attending and consulting) to ensure sharing and evaluation of pertinent diagnostic results pre-surgery and post-surgery, as well as relevant discharge planning information with inpatient to assist in expediting medical decision making. Coordinates with nursing and other team members to manage patients' clinical progression and commensurate plan for the efficient delivery of acute care services. Establishes cooperative and meaningful relationships with patient/family to set reasonable goals and expectations for acute care, recovery, and facilitates a safe and satisfactory transition to home or alternative post-acute facilities Conducts a comprehensive pre-surgery patient education program to ensure that, from the pre-surgery phase through the acute phase, proper care is in place and consistently delivered to all patients. Assists in the development and implementation of patient care protocols, clinical pathways, and in updating policies and procedures for bariatric surgery patients. Assists the program coordinator in managing accreditation criteria to ensure clinic/facility/program meets expectations set forth by governing agencies. Maintains and ensures compliance for all policies and procedures that guide and support the assigned services. These include, but are not limited to: Joint Commission requirements, federal and state regulations, MBSAQIP standards, regulatory standards, etc. Develops, monitors, analyzes, and reports program performance improvement indicators and initiates process improvement as needed, while maintaining an ongoing QI plan. Leads and organizes the MBS committee meetings and facilitates quarterly physician review of quality, safety, and patient outcome data every two weeks as directed by the program coordinator. Provide appropriate care specific to the age of the patient to ensure understanding and comfort level of treatment as outlined in the "Age Specific Criteria". Demonstrates ability to identify opportunities for and give individualized customer service/patient care according to customer/patient needs. Right Task. Right Person.
