RN-Client Services 20-40 hours available Salary Range (dependent on previous experience)- $28.00-$36.00 per hour Job Summary: Responsible for the assessment, activity planning, case management, and direct nursing services of individuals receiving services in the Adult Day Center. This position is also responsible for advising on clinical operations of the Adult Day Service, Homemaking, Personal Care, and National Caregiver Support programs of Licking County Aging Partners (LCAP). Specifically, this position will be responsible for overseeing training for LPNs, LSWs, and direct service providers within the specified program areas. Personal & Professional Attributes: Demonstrate sensitivity, empathy, and understanding of the needs of older, impoverished, and developmentally disabled individuals, display common sense and good judgment, and actively promote LCAP to the public. Uphold the highest level of confidentiality, honesty, and integrity, and represent the organization in a positive and professional manner at all times. Core Technology Competencies: Demonstrated working knowledge of computer operations, standard office equipment (copiers, faxes, etc.), and must be able to utilize all Microsoft Office services. General Requirements: Commitment and desire to provide excellent service to all clients and aging community members. Work stated hours, communicate, comprehend, and follow policies and procedures, and develop and maintain positive working relationships with all staff. Must be able to satisfactorily perform all the job responsibilities and meet the physical requirements of that same job title. All employees must possess and maintain a Valid Ohio Driver’s License, as well as the minimum auto insurance coverage for Ohio (must be able to provide proof upon request). Requirements Minimum Education and Experience Requirements: Ohio RN licensure in good standing. CPR certification. Experience evaluating and coordinating health or social services within home and community-based settings. Knowledge of case management and referral principles and practices. Knowledge of local resources and programs available to maintain the health and welfare of older adults and those with developmental disabilities. Knowledge of current issues related to caregiving. Must successfully pass a background check and pre-employment drug screening. Collaborate with Client Services Manager to develop initial and ongoing learning plans for staff providing direct care in the Adult Day Center and within the community. Administer skill testing for direct service providers and LPNs as needed. Conduct assessment, activity planning, case management, and direct nursing services within the Adult Day Center. Serve as the nurse on duty when there are client-related health emergencies in the Adult Day and Community centers. Enter and maintain information to ensure a complete, accurate, and informative client record. Review assessments and approve service plans for clients receiving Homemaking, Personal Care, and National Caregiver Support. Model high-performance standards characterized by integrity, and to earn the trust and respect of others by coaching, inspiring, and empowering teams of people to achieve strategic objectives. Develop internal and external communications with tact, finesse, and confidentiality according to LCAP policies and procedures. Perform other related duties as needed or as assigned.
