This position is a fully remote role, including orientation, with occasional onsite requirements. The role involves providing patient care support for a centralized nursing program, outpatient and home care services, including triage services, anti-coagulation, and refill. Individuals in this role will work in an outpatient clinic setting.

Requirements

• Associate's or Vocational degree in nursing.
• Minimum 3 years RN experience.
• Licensed Registered Nurse-MN Board of Nursing required.
• Licensed Registered Nurse-WI Dept of Safety & Professional Services required by completion of orientation.

Nice To Haves

• Experience in triage, anticoagulation, or remote nursing support.

Responsibilities

• Collect, prioritize and synthesize comprehensive data pertinent to the patient's health or situation.
• Analyze assessment data to determine nursing diagnoses.
• Identify expected outcomes individualized to the patient.
• Develop a plan that prescribes interventions to attain expected outcomes.
• Implement the identified plan and document interventions according to guidelines.
• Evaluate the patient's progress towards attainment of the outcome.
• Participate in quality improvement activities related to nursing practice.
• Engage in self-evaluation of practice on a regular basis.
• Collaborate with patient, family, and others in the conduct of nursing practice.
• Act in an ethical manner and maintain a therapeutic patient-nurse relationship.
• Integrate research findings in practice.
• Provide leadership in the professional practice setting.
• Practice in an environmentally safe and healthy manner.
