About The Position

LewisGale Hospital Montgomery is seeking an experienced RN Case Manager to ensure exceptional care for patients. The role involves developing case management plans for new and recurring patients, evaluating admissions for medical necessity, performing utilization management reviews, and acting as a liaison between patients, families, and healthcare providers. The position emphasizes patient care and collaboration within a supportive team environment.

Requirements

  • Diploma or Associate's Degree from a professional nursing program (required).
  • BSN or current enrollment in a BSN program at an accredited school of Nursing (preferred).
  • Current licensure as an RN in the state of Virginia or current multi-state compact RN license with eligibility to practice in VA (required).
  • American Heart Association Basic Life Support (BLS) Health Care Provider certification (required).

Responsibilities

  • Develop a case management plan of care for new and recurring patients, including clinical, psychosocial, and discharge needs.
  • Evaluate admissions for medical necessity using approved criteria at defined intervals throughout the episode of care.
  • Perform utilization management reviews and communicate information to third-party payers.
  • Act as a liaison through effective and professional communications between physicians, patients/families, hospital staff, and outside agencies.
  • Make appropriate referrals to third-party payer disease and case management programs for recurring patients and patients with chronic disease states.
  • Assess patients' post-discharge needs and facilitate the provision of necessary services.
