The Registered Nurse Case Manager is a hybrid field-nurse/manager role responsible for overseeing long-term care for patients in Lorain and Medina counties. This position involves planning, organizing, and directing home care services, ensuring quality patient care through the development and evaluation of individual care plans. The role requires strong communication skills as the Case Manager acts as a liaison between patients, families, and healthcare teams, while also adhering to established professional standards and guidelines.

  • Complete assessments of patient and family to determine home care needs.
  • Provide a complete physical assessment and history of current and previous illnesses according to physician's orders.
  • Update the plan of care regularly as necessary and document care following CBNS procedures.
  • Include the patient and family/caregivers/staff in the planning process.
  • Initiate appropriate preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed by the physician.
  • Provide health care instructions to the patient as appropriate per assessment and plan.
  • Deliver competent, quality patient care in the home.
  • Maintain communication with Clinical Manager, CBNS Staff, and all other treatment team members.
  • Provide skilled nursing care visits that conform with CBNS patient care and general medical policies.
  • Participate in Case Care Conferences as needed.
  • Maintain continuing knowledge of competencies related to the Nursing profession through education programs and workshops.
  • Participate in on-call rotations with other case managers as scheduled by CBNS.
  • Ensure arrangements for equipment and other necessary items and services are available.
  • Instruct, supervise, and evaluate LPN care provided when assigned by the clinical manager.
  • Perform OASIS assessments and medication audits.
  • Prepare clinical notes and update the primary physician as necessary.
  • Communicate with the physician regarding the patient's needs and report changes in the patient's condition.
  • Registered Nurse currently licensed in the State of Ohio.
  • Minimum of one (1) year experience as a licensed, registered nurse, preferably three (3) years including home care clinical experience.
  • Preferred experience in a supervisory or quality assurance position in acute and/or home health.
  • CPR certification.
  • Valid driver's license.
  • Excellent observation, verbal and written communication skills.
  • Problem-solving skills and basic math skills; nursing skills per competency checklist.
