About The Position

Join Iowa Home Care as a Registered Nurse Case Manager, where you’ll play a crucial role in providing compassionate, patient-centered care directly within clients' homes. Requirements

  • Graduate of an accredited school of nursing.
  • One (1) to two (2) years of recent acute care experience in an institutional setting.
  • Current licensure in state and CPR certification.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  • Prolonged or considerable walking or standing.
  • Able to lift, position or transfer patients.
  • Able to lift supplies and equipment.
  • Considerable reaching, stooping, bending, kneeling or crouching.
  • Visual acuity and hearing to perform required nursing skills.
  • Licensed driver with automobile that is in good working order and insured in accordance with the organization requirements.

Responsibilities

  • Conduct initial and ongoing assessments of patients and their families to determine home care needs.
  • Develop and update individualized care plans based on health assessments, including therapeutic, preventive, and rehabilitative actions.
  • Administer medications and treatments as prescribed, provide skilled nursing care per the State Nurse Practice Act, and implement preventive and rehabilitative procedures.
  • Offer guidance to patients and families to help them understand and manage their health.
  • Coordinate care across services and ensure seamless support.
  • Collaborate with physicians and community health providers to optimize care plans and outcomes.
  • Maintain accurate and timely clinical notes, updating the primary physician at least every 60 days or as needed.
  • Communicate any changes in patient status to the physician and obtain necessary orders.
  • Provide health instructions tailored to the patient’s needs and care plan.
  • Include families in the planning process to ensure they are well-informed and supported.
  • Instruct, supervise, and evaluate home health aides at least bi-weekly to ensure quality care delivery.
