About The Position

Crescent Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join their dedicated nursing team, providing exceptional end-of-life care throughout the community in Dillon, Marlboro, and Chesterfield County. This rewarding role involves delivering hands-on hospice nursing care, coordinating services, and guiding patients and families through every step of their hospice journey. As a valued member of an interdisciplinary care team, the RNCM will help ensure every patient receives care grounded in compassion, clinical excellence, and connection. Requirements

  • Hold current unencumbered license as a Registered Nurse.
  • Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
  • Must be computer proficient in typing and various programs, including background in EMR.
  • Possess and maintains current CPR certification if required by state.
  • Current driver's license, vehicle insurance, and access to dependable transportation.
