About The Position

The Staff Pad has partnered with Russellville Hospital in Russellville, AL to hire a dedicated RN Case Manager. This role is responsible for coordinating and managing patient care throughout the hospital stay, ensuring quality, efficiency, and safe transitions to home or post-acute care. The RN Case Manager works closely with physicians, nursing staff, patients, and families to optimize outcomes and facilitate smooth, patient-centered care.

Requirements

• Current Alabama RN license
• Minimum 3 years of hospital experience in Med-Surg, Case Management, or related setting
• Strong communication, critical thinking, and organizational skills
• Knowledge of care coordination, utilization review, and discharge planning processes

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred

Responsibilities

• Assess patients and develop individualized care plans in collaboration with the healthcare team
• Coordinate patient care across interdisciplinary teams to ensure continuity and quality
• Facilitate discharge planning, including referrals to home health, rehab, and community resources
• Monitor and track patient outcomes and identify opportunities for improvement
• Educate patients and families regarding diagnoses, treatments, and follow-up care
• Ensure compliance with hospital policies, regulatory standards, and documentation requirements
• Act as a patient advocate, addressing questions and concerns throughout hospitalization
