About The Position
RN Case Manager PRN Mileage Reimbursement Central Ohio area National Church Residences Home Health division located in Central Ohio is a Home Health & Hospice agency with a team of health care professionals that is unmatched in the area! We are owned and operated by National Church Residences , the nation’s largest provider of affordable senior housing and health care services. We are seeking a RN Case Manger who will share in our vision to advance better living and care for seniors! This person will do mostly admissions. Position Summary: We are currently seeking a dedicated and compassionate Registered Nurse Case Manager to join our Hospice team. The ideal candidate will embrace our mission and provide skilled nursing care while coordinating the overall care plan for hospice patients and their families.
Requirements
- Graduate of an accredited nursing program with a current RN license.
- Minimum of two years of RN experience required.
Nice To Haves
- Previous hospice or home health experience is strongly preferred.
