About The Position
RN Case Managers are a critical part of the agency and must have experience working in the home health setting. We are looking for individuals that are versatile and dependable. Individuals must have the following: Must hold a current license as a Registered Nurse from the Virginia State Board of Nursing. Must have experience working in a home care agency.
Requirements
- Must hold a current license as a Registered Nurse from the Virginia State Board of Nursing.
- Must have experience working in a home care agency.
Responsibilities
- Complete initial and ongoing comprehensive assessments for client admissions.
- Create patient plan of care and reevaluate as necessary.
- Identify any additional patient needs and facilitate appropriate referrals.
- Complete clinical nurse notes in their entirety.
- Facilitate monthly home visits for skilled care.
- Additional duties as assigned.
