Why You’ll Love This Role The Hospice Registered Nurse Case Manager ensures quality and safe delivery of hospice services. Plans, manages, coordinates and/or delivers services provided in the home and in facilities utilizing computerized documentation system and telehealth. Registered Nurse Case Manager will provide hospice patients of Emblem Hospice with comprehensive care while maintaining continuity of care with all disciplines involved. This position will reflect the agency mission and values at all times, while adhering to standards, rules, and regulations for hospice.

Requirements

  • Registered Nurse licensed in the State of Arizona.
  • I-9 documents verifying citizenship and/or eligibility to work in the United States.
  • Certified in Health Care Provider CPR
  • Negative Tuberculosis skin test/chest x-ray results within a year, with annual verification of continued negative results.
  • Valid Arizona Level 1 Fingerprint Clearance Card
  • Automobile in good working condition, Current Arizona Driver’s License, & current auto insurance for vehicle being used.
  • Technical aptitude to use a hand held electronic device to perform documentation, send/receive email, and perform other tasks using the Internet, word processing software, and other programs.
  • Possesses effective written and verbal communication skills.
  • Organized and able to multi task and set priorities
  • Self direction and ability to work productively with minimal supervision.

Nice To Haves

  • One year of hospice nursing experience (preferred).

Responsibilities

  • Provides hospice nursing care according to the Interdisciplinary Group (IDG) Plan of Care that supports the hospice philosophy by assuring quality of life is maximized and pain and discomfort, physical and spiritual, are minimized.
  • Ensures that patients are hospice-appropriate. Upon receipt of referral, contacts patient and/or family to introduce self and to confirm initial appointment.
  • Performs initial assessment and evaluation, as well as ongoing weekly or more comprehensive nursing assessments to meet the needs of the patient.
  • Initiate appropriate skilled, supportive, and appropriate nursing procedures, including abatement of pain and distressing symptoms according to the IDG Plan of Care.
  • Assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process.
  • Develops implements, evaluates and updates treatment plans on identified patient and family needs.
  • Communicates regularly with Hospice Clinical Director and/or Market Leader to provide updates on patient, family, or facility concerns.
  • Posts a Visit Calendar in the patient's home with information on when visits are planned and by whom, when appropriate.
  • Provides patients and families with education related to care, safety, disease process and, when applicable, hospice philosophy and care.
  • Educates facility staff on hospice philosophy and care when patients are Residents of any facility.
  • Communicates professionally with the physician and keeps the physician informed of any significant change in patient status. Documents such according to Emblem Hospice policy.
  • Attends IDG Meetings, in-services and meetings as required.
  • Completes timely fulfillment of training, license renewals, and certification renewals according to company policy and regulatory requirements.
  • Initiates interdisciplinary referrals when appropriate and coordinates scheduling of visits of all disciplines for the benefit of the patient.
  • Supervises and documents supervision of visits of Home Health Aides and Licensed Practical Nurses.
  • Will provide guidance and direction to RN and/or LPN Support Nurses and RN and/or LPN On Call Nurses.
  • Schedules patients with other RNs or LPNs on days off. Responsible for communicating specific instructions to On Call and PRN RN’s for continuity of care.
  • Initiates and revises Hospice Plan of Care, with input of Hospice IDG team as patient’s needs require.
  • Responsible for legibility, completeness and accuracy of patient chart. Documentation is individualized to patient and every effort is made to complete all documentation in the Electronic Medical Record by the close of shift. Notification is required to supervisor if issues arise.
  • Scheduled visit frequency is appropriate to patient/family needs and schedule. Responsible for meeting the agency productivity standards for RN.
  • Submits documentation in a timely manner per agency policy.
  • Responsible for ordering appropriate and sufficient supplies for patients and making sure the supplies are in the patient care area of the home. Responsible for delivering supplies to the patient’s home or arranging for the supplies to be delivered.
  • Responsible for awareness of patient's personal schedules and communicates relevant information to all who may be affected.
  • Acts as a liaison with family and other caregivers. Counsels patient and family in meeting nursing and related needs. Provide appropriate support at time of death and perform bereavement assessment if requested.
  • Participates in rotational on-call assignments as required.
  • Utilizes computerized clinical records system and other technologies provided by the agency.
  • Participates in Quality Assurance Activities as assigned. Conducts and presents self in a professional manner. Uses equipment and supplies appropriately, with attention to proper maintenance and care. Utilizes company resources appropriately.
  • Notifies agency administrator and supervisor of any suspicions or observations of abuse, fraud, or waste, or violations of any patients’ rights involving any patient, family member, or employee of Emblem Hospice, Inc. This also includes notifying the administrator in any facility the aide is working in of the same concerns involving facility patients, their family members, or employees.
  • Actively participates in quality assessment performance improvement activities.
  • Understands and embraces the Emblem culture and seeks ways and opportunities to demonstrate the values in all aspects of performance, behavior, and interpersonal communications.
