RN - Case Manager - Canton - HSNNY
About The Position
Health Services of Northern New York is seeking a Registered Nurse Case Manager to support Canton and the surrounding areas. This position will be responsible for the management of client care in the home setting. The Registered Nurse Case Manager will perform nursing assessments, coordinate care with physicians, and have oversite of additional discipline involvement with each client. Additional discipline involvement could include Physical Therapy, Occupational Therapy, Speech Therapy, Medical Social Worker and Home Health Aides. This position will require travel to and from patient homes throughout their specified region. An ideal candidate will have excellent communication skills and be comfortable traveling to and working one-on-one with patients in their homes.
